Our client supports 100's of vulnerable people in the community that have Learning Disabilities and other complex needs. We are currently looking for an experienced Manager with the management of the Domicilliary Care Services within the organisation. You will manage 7 units including Supported Living and Sheltered Accomodation and be responsible for 7 staff and 12 service users that have Learning Disabilities, Autism, Aspergers and Mental Health You will have a good knowledge of CQC standards aswell as implementing policies and procedures to suit CQC requirements. You will have strong management skills with a passion for troublshooting, you will need to have an NVQ 4 in Care Management and knowledge of Learning Disabilities and Mental Health. You will need to be flexible towork a rota and also have a strong personality with clear communication skill both verbal and written. TXM Healthcare have a exciting opportunity for the post of Service Manager for the Psychiatric Intensive Care Unit based in London. This is for 15 bedded PICU ward for women who require intensive support during an acute phase of their illness This post focuses on clinical management and leadership to develop and deliver robust therapeutic care and management of service users, who are acutely high risk to self and/or others. The post holder will also work with other service managers across the hospital as required to provide advice and support in relation to the care needs of service users on other wards. The successful candidate will have at least 2 years' experience at team leader level or above (band 6 equivalent) and will be a highly self-driven and motivated clinical leader who is able to lead and manage the challenges of a PICU. The post holder will ensure appropriate pragmatic risk management support procedures are developed and routinely applied in the intensive care service for the identification and management of risk. Responsible for managing clinical performance and manage operations within the service, in accordance with Company policies and procedures. To manage operational resources in a cost-effective manner, evaluating & ensuring the standard of service delivered is commensurate with the company's vision and regulatory standards. To co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the hospital, to ensure effective interface and continuity of care between disciplines and services. It will be an advantage if you have experience of working in a PICU and/or managing a diverse nursing workforce. You will also have the mentorship in practice or ENB 998. Main Purpose of Job: *To assess, plan, implement and evaluate nursing care to individuals being prepared for discharge to a community/domiciliary setting, having regard to the responsibilities of being the designated caseload holder during the Discharge process. *To facilitate the safe discharge and transfer of clients to the companies care within designated timescales *As part of the multi disciplinary team ensure safe and appropriate handover of care is given to bespoke care teams recruited for discharged clients. You must have strong community experience, recent experience preferred, be a car driver, ideally living in between London and Brighton for easy access to major road links. Due to the nature of the role early starts and late finishes maybe required due the travel required in the role, plus overnight stay's away from home due to training requirements of new teams. To be considered for this position you must have a registered general nursing degree currently registered with NMC and show proof recent CPD, also recent experience in the community working with inter agency services for vulnerable adults or children is desired. Extensive experience of assessing and providing care to people with complex and long term conditions within the community.
