Clinical Nurse Specialist - Youth Offending Service Our client has a job vacancy for a Clinical Nurse Specialist to work within a Youth Offending Service (YOS) based in North East London. This is a full time position with an initial 3 month contract and the pay rate for this role is 30-32 per hour (limited company worker). Requirements - All applicants must be a first level registered RMN with a valid NMC Pin, as well as 2 years recent post qualifying experience working with Young Offenders.

Our client has a strong record of delivering high quality supported living services for adults with learning disabilities. They are a not-for-profit organisation and a registered charity firmly established in the East Midlands, Wales and Shropshire.

Position: Support Worker
Location: Huthwaite, Nottinghamshire
Salary: £13,654 to £15,689 per annum (pro rata for part time posts) depending on skills and experience, with additional payments for sleep-in shifts.

Hours: Standard working week is 38 hours (Part Time opportunities will be available)

ROLE:
Our client is looking to recruit a team to support 3 men in setting up their own flats in the Huthwaite area. While developing as a new team you will provide a high standard of support that enables them to maintain their tenancies, learn new skills, promote their independence and develop valued roles within the community.

The Support Worker will work as part of a team to consistently deliver person centred services for people with a learning disability and to take responsibility for supporting them with the development of individual plans and risk assessments. Undertaking a key role in ensuring their participation features in all aspects of service planning and contributing to the development of the staff team by role modelling and sharing good practice.

RESPONSIBILITIES:
a) Work with the person you are allocated to, to identify, set and review goals within the structure of the Person Centred Planning/Quality of Life system.
b) Write skill development and opportunity plans, support people to take risks and develop plans that help the person manage risks and promote independence.
c) Liaise with family, advocates and professionals, develop communication strategies to help people make decisions.
d) Help people to develop (and maintain) personal friendships and relationships, and be active family members.
e) Review support plans and measure progress, support individuals in line with plans in place.
f) Guide to develop budgeting skills, help people to liaise with the Benefits Agency, landlords etc and with their finances in line with individual plans.
g) Support people to lead healthy lifestyles and promote good health, provide help for people to plan medical appointments and keep detailed and accurate records of health needs.
h) Liaise with class tutors, employment agencies, career advisors and day services managers, to help people take part in occupational and development opportunities.
i) Help people to utilise their local facilities and move away from segregated activities, support people to develop (and maintain) leisure pursuits and personal hobbies, support individuals in leisure pursuits and access to community based activities.
j) Develop communication profiles to improve their understanding of people communication methods, support people to actively participate in their home, including their domestic tasks.
k) Act as shift leader in absence of Community Support Manager or Assistant Manager, to undertake any training and development initiatives identified by the Community Support Manager or Regional Services Manager, undertake any other duties commensurate to the job role.

ESSENTIAL SKILLS / EXPERIENCE:
a) Successful candidates will have a positive attitude to trying new things and a commitment to helping people assess risks and solve problems.
b) You would also be able to act as a positive role model and have excellent verbal communication skills.
c) It is essential that you are highly motivated, dedicated and enthusiastic and can work within a team to provide structured support in a calm, person focussed way.
d) Successful candidates will need a basic level of fitness in order to be able to undertake their accredited physical management training.
e) The prospective tenants have expressed a preference for staff who can share their interest in football, can support them in developing gardening skills and managing their finances.
f) They also want a staff team that can adapt their communication styles to meet their individual needs.

DESIREABLE SKILLS / EXPERIENCE:
a) An understanding and experience of individuals on the Autistic Spectrum and who require intensive support with complex behaviour would be an advantage.

Key Responsibilities:- To supervise individuals with the Home, ensuring they are fully involved in the Homes' activities Observe and record the progress of individuals and report to senior management Care for individual's property and undertake and aspect of Home management including housekeeping, meal preparation and laundry Assist in the development of practical skills including shopping, visiting the doctor and dentist Assisting with self-care as required to maximise domestic skills and independent living Participate in the development and implementation of opportunities for recreation, socialisation and holidays Attend staff meetings to discuss and review training progress This list is by no means exhaustive. Lead Transitional Support workers required. This position would suit social workers who are training and are looking for work placements. 37.5 hours per week to include 1 sleep each week Pay scale between �7.00 to �8.50 depending on experience and qualifications. This could be negotiable for the right person. Experience and relevant training and qualifications are essential Must have good communication skills and be able to produce good written work. You will need to be conversant with CQC essential standards of quality and safety. Ideal person will need to be confident in their role and be able to be a lead person for key working responsibilities. Must have a good understanding of working with vulnerable adults on the autistic spectrum and be able to manage challenging behaviour in a calm and positive manner. Preferably experienced with working with young adults with learning difficulties and complex emotional needs between the age of 18 to 25.
